Acromegaly is a rare disease being followed up by primary care team. It usually caused pituitary adenoma and the treatment would be resection of tumour. But surgical intervention end for acromegalic patient? This case highlighted continuity in 65-year old gentleman who had been diagnosed acromegaly since age 31. He undergone transseptal transsphenoidal his when he was 45-year-old. Postoperatively, there were still residual growth hormone (GH) insulin like factor (IGF-1) secretions. However, during 20 years follow after operation, despite increasing level GH IGF-1, management neglected as focus shifted towards other medical problem, which diabetes atrial fibrillation. During latest up, complaint unspecified headache expressed worry regarding recurrence condition.
